I am calling the Ombudsman..to investigate my husband situation..tried to talk today sunday 24 of May to the nurse supervisor ( voice sound philipino who usually are very polite)..to explain him about a diabetic condition of my husband who has SOMOGY..for which I took him to a Japanese hospital where they did understood and know this condition..this is a neurological response to near diabetic coma…it reverse his sugar internally..but is an unstable response..YESTERDAY MY HUSBAND WAS 32..at 11pm..this facility DID NOT CALL THE AMBULANCE..as they should..to cover up their mistake he was keept..injected..waited until his glucose was 73..waited more until he was 109 ..THEN THEY CALLED 911…and this facility IS CROSS THE STREET OF SUTTER HOSPITAL ==>EMERGENCY WHO EVER IS THE OWNER OF THIS FACILITY…YOU NEED TO INVESTIGATE YOUR EMPLOYEES..the supervisor i tried to talk to..(today arround 1pm)..was not only rude..but verbally abusive..he would not let me explain the VITAL condition..my husband needs to be checked more frequently…he would talk over me SO WOULD NOT HEAR ME.. i started to get nervous..teary..he raised his voice and interrupted me again..THAT WHY WAS ACTING LIKE THAT..as if you have not right to express my concern..brutal…and ignorant..i said can i talk now..explained he needs to be checked often..he said now 4 times..which meant..he care non about the medical case condition..finally he said are you done now..and hang up..I was in distressed and call an extended family..she will call them..she is also diabetic..critical case also..she said the facility needs to be reported for neglect ..my husband could have died..they cover up..never call me to informe..me.. SO I CALLED THE OMBUDSMAN Contra Costa..1 707 631 7982..we called her before when the 5 thousand dollar insuline pump was lost at this same facility and wanted us to pay for it..they had to replaced it …for the pump he has now.. Not working well..reason why last night his insuline was 32…WHO EVER OWNS THIS PLACE..YOU HAVE A PROBLEM WITH THE EMPLOYEES YOU ARE HIRING…HIRE PEOPLE THAT FOLLOW THE LAW…foreign employees..do not seem..to undertand…their missteps..that can cause death.. SO I AM CALLING THE OMBUDSNAN

Great staff very helpful and they care about the patients

The staff is wonderful. I greatly appreciated the care they gave my loved one. Grateful for how the administrative staff helped us navigate through difficult decisions.

Since the new remodel was completed this place has been top notch. No negative feedback whatsoever.

The only logical explanation for those glowing reviews is that they’re from employees or their friends—because this facility is an utter nightmare. Four patients crammed into a room no bigger than a prison cell, separated only by thin curtains. The call buttons? Useless. You’re left waiting indefinitely. My grandfather had an accident—wet himself—and staff actually told me he’d only pushed the button ten minutes earlier. Left him sitting there in urine, tossed a napkin over his lap like that fixes anything, then vanished for another ten while he lay exposed. I thought maybe he was exaggerating to guilt me into springing him… until I walked those halls myself. Smells hit you everywhere—bedpans, sweat, decay—you can’t tell if it’s furniture or flesh rotting. And everyone working there? Hollow. Dead inside. I’d sooner let my worst enemy rot on the street than send family here. Shut it down.

I had GREAT OCCUPATIONAL & Physical THERAPY you could ask for!! All of the therapist were OUTSTANDING!! They’ll treated you like you were their own family!! If I was ever in the position where I had to go stay somewhere I would go back Lone Tree Acute Rehab on Lone Tree Way,Antioch. The food was good n so were the nurses!

This place is the Best! Very caring, thorough and clean. Every Department has been excellent in their capacity. Some go above and beyond and it is done with loving care. The entire staff has been most hospitable. Would highly recommend this facility to anyone needing rehab. Have noticed even the long term care folks are well taken care of! A few extra special people will be forever remembered in our journey here. PT, OT, CNA’s, Nurses, Med Techs, Social Services staff, Activities Department, Housekeeping, Maintenance and the food is very good. Many Thanks for the special people who worked with my mother in getting her well. I rate this facility a 10+

One of famliy member stayed here early this year. We were so worry at the beginning cause the horrible reviews on Yelp. Our case manager (Brenda Riley) was very helpful. First of all, We have a face to face round table meeting with Case manager, Social worker, etc, about 7-8 people. They explained every step and benchmark, what should we expect. They even asked if the patient more toward sweet, so they can give him a little treat each meal to make him feel like home. The facility is clean, the room is clean. toilet is spotless. The Physical therapists are pretty and patient. The nurses(both male & Female) are yound, strong, and friendly. Each time we visit, Nurse let us know how much the patient ate on the day. Our family member was happy to stay there and had a speedy recovery. Brenda Riley monitors his progress, and communicated with hosipital for any equiment / med update. We have a great experience !!!!

The facility was clean, well-maintained, and had a warm, welcoming atmosphere. I can’t say enough about the leadership at this location, and the treatment you will receive.

My experience here was outstanding. The entire team — from nurses and therapists to the administrative staff — truly cared about the patients’ recovery and comfort. The nursing staff was attentive and professional, always quick to respond and very patient when explaining treatments or medications.

I cannot thank lone tree post acute enough for the love and care they gave my dad during his time there. Before his passing, the staff truly became like family to us. They treated him with the utmost respect, dignity, and kindness every single day. It gave us so much comfort knowing he was surrounded by people who genuinely cared for him. We will always be grateful for the compassion and support they showed not only to my dad, but also to our whole family during such a difficult time.

Nurse are very great. Physical therapy very wonderful and very patient. Overall everything was very good.

My father was here where we hoped he would recover his mobility. That didn’t happen, but while he was there I was very happy with the care he received. The staff were extremely patient with him which wasn’t always easy. My father was a very independent man and didn’t always adjust well to having to rely on others. Along with the medical treatment and physical therapy, he got haircuts, played plenty of bingo and participated in other activities. There was a courtyard we went to on sunny days which he really appreciated. It was no fun not having him at home and I went to see him every day. This may have contributed to the way he was treated, but I saw the same level of patience and care with people there who never had visitors.

Highly recommend this facility. I have had a loved one here for the last few months, and the staff has been exceptional. The nursing team is very attentive and the therapy staff is amazing. The facility is always clean, and the food is great! Thank you Lone Tree for the excellent care!

The staff are very professional! Always going the extra mile to help the residents. I know folks who have been there for recovery and they wanted to stay longer because of how well they are treated by staff & volunteers. They love the place! They are kind and patient. This is why I give them 5 stars!

I can’t say enough good things about Lone Tree Post Acute in Antioch. The staff here are truly amazing—compassionate, attentive, and always going above and beyond to make residents feel comfortable and cared for. The activities department is also top-notch. They consistently plan engaging and meaningful events that bring joy and connection to the residents. It’s clear that the team genuinely cares about enhancing the quality of life for everyone here.

I’ve heard nothing but positive things about this facility. Very well staffed, clean environment, lots of activities to keep residents active.

My brother’s and I had our mom placed here. Her stay was 3 weeks . OT and PT thought she was ready to come home, so our insurance kicked her out. Wish she could have stayed another week to continue to strengthen herself. She was sent home with a Care team….we weren’t left hanging. The staff was great and the CNA’S were attentive. I would recommend this place and I would strongly recommend going for a visit before placing your loved one…I did just that! Because the reviews gave me mixed feelings.

I have been here for some years now and always get great care with people like Balwinder, Kamaljit, Lauren, Reggie and Mueller. Patricia who helps me with my appointments. Maria the RNA who helps me work on being mobile and has patience.

I want to thank Patricia who has been a phenomenal help to getting me the appointment I need to get my arm back to normal! This place really took great care of me and I come back whenever I need the help after a hospital stay. They have amazing floor nurses and cnas like Chastity. Anne in activities always keeps residents entertained. Therapy team is always happy to help us like Radlyn. Thank you Lone Tree!
